Health Requirements & Communicable Diseases Policy

To participate in clinicals, students must submit all required health and medical documentation to our third-party online document manager. Students are responsible for scheduling and paying for all appointments, lab work, and immunizations.

Required Documentation:

  • Medical examination (physical) completed by a Primary Care Provider (Physician, Physician Assistant, Nurse Practitioner) within 6 months of the start of class.
  • COVID-19 vaccines: Documentation of full vaccination (booster may be required).
  • Tuberculin (TB) Test: Two-step PPD or QuantiFERON TB Gold.
  • Tetanus/Diphtheria/Pertussis: Tdap or Td vaccine administered within the past 10 years.
  • Hepatitis B series or Hep B SAB: Series is strongly recommended.
  • MMR (Measles/Mumps/Rubella): Positive titer or immunization/boosters.
  • Varicella (chickenpox): Positive titer or immunization/boosters.
  • Influenza (flu shot) for the current flu season.
